Heart Failure Pathophysiology & GDMT Mechanisms

In heart failure, reduced cardiac output triggers compensatory RAAS and sympathetic nervous system activation. Initially adaptive (maintaining perfusion), chronic activation drives vasoconstriction, sodium retention, volume overload, myocardial fibrosis, and progressive cardiac remodeling. Three of the four GDMT pillars target this axis.

Cardiac remodeling — progressive changes in ventricular size, shape, and function — is the central pathological process in HF progression. Initial injury (MI, pressure overload, toxins) triggers compensatory hypertrophy that becomes maladaptive. Chamber dilation, wall thinning, spherical geometry, and mitral regurgitation create a self-perpetuating cycle.

Congestion is the primary driver of HF symptoms and hospitalizations. Sodium and water retention (RAAS, ADH, impaired renal perfusion) leads to elevated filling pressures, pulmonary and systemic congestion. Hemodynamic congestion often precedes clinical congestion by days. Diuretics provide symptomatic relief but do not modify disease progression.

The 2022 guideline established four simultaneous pillars of GDMT for HFrEF: ARNi (or ACEi/ARB), beta-blocker, MRA, and SGLT2 inhibitor. Each targets a distinct pathophysiological pathway, and their benefits are additive. Combined, they reduce mortality by approximately 60-70% compared to no therapy.

ICD prevents sudden cardiac death from ventricular arrhythmias, CRT restores mechanical synchrony in patients with LBBB and wide QRS, and LVAD provides mechanical circulatory support in advanced HF. Device selection depends on LVEF, QRS morphology, NYHA class, and GDMT optimization status.
